I'm a Corporate Member. How do I locate my previously issued Unique User ID and Password? Your Unique User ID and Password were included in the EHE benefit information mailed to your home address. New Corporate Members or first time users of online appointments will be asked to change their Password after initial log-in. Be sure to record both your Unique User ID and Password safely and securely as this is your access to your Medical History and your EHE Personal Health Record. I'm a private patient and a first time user of online appointments. How do I obtain a Unique User ID and Password? A Unique User ID and Password can be created by following the New User link above. Will I receive an appointment confirmation? Yes! Within minutes you'll receive an email confirmation with your Appointment Confirmation number. Please record this number for future reference. |
How can I access my appointment confirmation information? You can view, change, or cancel appointments with your EHE Appointment Confirmation number. Simply log-in with your Unique User ID and Password and follow the instructions. Should you change or cancel your confirmed appointment, a confirming email will be issued within minutes. Please record this information for future reference. |
I need to complete my online Medical History. How do I access this form? Once you've received an appointment confirmation, you may access your Medical History with your Unique User ID and Password. Be sure to "Save" each completed page to avoid loss of entered information. Once completed, click "Submit" and your Medical History is on its way to your EHE physician! There's no need to bring a copy with you to your appointment. |

A word about canceled and missed appointments. Our physician and clinical staff time is scheduled based on your availability. For this reason, canceled appointments require a minimum of two business days' notice to avoid a cancellation fee. Missed appointments by Corporate Members will be billed to their employer at full exam value. Missed private patient appointments will be charged to your credit card at full exam value. Our Resource Directors are available to answer your questions about canceled and miss appointment fees. |
